We are seeking a skilled Licensed Practical Nurse to join our team in the Surgical Care Service at the Boston VA Healthcare System.

The successful candidate will work under the direction of a PATC and report to the PATC Manager. As a member of the Patient Aligned Care Team (PACT), the LPN will demonstrate active collaboration and communication with the core Team and extended Team, including family/caregiver, clinical pharmacist, dietician, social worker, behavioral health specialists and other internal and community-based services and disciplines involved in providing care to the patient.

The LPN will be responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN will provide patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's personal goals, including preparation for surgical procedures.

Work Schedule: Monday-Friday 7:00a.m.- 4:30 p.m.

This is a full-time position offering a competitive salary range of $78,183 - $101,640 per year, depending on experience and qualifications.

To be eligible for this role, you must have a current, full and unrestricted registration as practical/vocational nurse in a State, Territory, or Commonwealth of the U.S. or in the District of Columbia. You must also be proficient in written and spoken English.

The ideal candidate will possess comprehensive practical nursing knowledge and skills in specialized procedures and practices, serving as a resource person for other staff in organizing and implementing more complex/specialized patient care.
