Per Diem Nurse Practitioner

As a Per Diem Nurse Practitioner - Community Health Specialist with Matrix Medical Network, you will play a crucial role in improving healthcare outcomes for at-risk individuals. This position involves conducting comprehensive health assessments in various community settings, including homes, skilled facilities, and through telehealth visits.

Responsibilities:

• Conduct thorough health assessments, reviewing medical history, medication adherence, and social environments to identify potential health risks.
• Develop and implement personalized care plans to address identified health concerns and promote overall well-being.
• Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless patient care.
• Provide education and support to patients and their families on disease management, treatment options, and healthy lifestyle choices.
• Maintain accurate and detailed records of patient interactions and assessment findings.

Requirements:

• Master's degree in Nursing or related field; OR commensurate experience and satisfactory completion of NP licensure.
• Current RN and NP licensure in state of practice with prescription authority or ability to obtain.
• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
• Minimum 1 year of experience as a Nurse Practitioner.
• Strong computer skills and familiarity with electronic health record software.
• Excellent verbal and written communication skills.
