About the Role:

We are looking for an experienced nurse with a strong background in patient care delivery to work collaboratively with physician partners to optimize quality and efficiency of care for hospitalized members. As an Inpatient Case Manager, you will be responsible for carrying out daily utilization and quality review, monitoring for inefficiencies and opportunities to improve care, developing safe discharge plans, and recommending alternative levels and sites of care when appropriate.

Key Responsibilities:

  • Develop and implement individualized plans of care to ensure continuity, quality, and appropriate resource use.
  • Review and monitor patients' hospital stays to assure timely and efficient delivery of essential services.
  • Collaborate with physicians, multidisciplinary healthcare teams, and patients/families to develop and implement effective discharge plans.
  • Assess high-risk patients in need of post-hospital care planning and coordinate implementation of discharge plans to meet identified needs.

Requirements:

  • Minimum two (2) years of experience in patient care delivery or completion of Masters degree in Case Management Program in lieu of minimum years of experience.
  • Associates Degree in Nursing or higher.
  • Registered Professional Nurse License (Georgia) and Licensed Registered Nurse (Temporary - Georgia).

Preferred Qualifications:

  • Minimum five (5) years of clinical nursing experience in a hospital setting.
  • Bachelors Degree in Nursing, Health Care, or Masters degree in Case Management.

Estimated Salary Range: $83,441 - $122,447 per year. Compensation may vary based on individual qualifications and market conditions.
