Job Summary

We are seeking a highly skilled Stroke Nurse Navigator to join our team at Tufts Medical Center. As a key member of our Comprehensive Stroke Center, you will play a vital role in providing exceptional patient care and education.

About the Role

This is a full-time position that requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care. You will work closely with a multidisciplinary team to ensure seamless transitions of care and provide ongoing support to patients and their families.

Key Responsibilities
  • Perform daily rounds on inpatients with stroke as a primary diagnosis
  • Develop and maintain relationships between all members of the CSC and Neurology Department
  • Participate in quality assurance initiatives to improve transitions of care
  • Conduct post-discharge patient follow-up phone calls
Requirements
  • Associate's degree in Nursing or higher
  • Registered Nurse (RN) license in Massachusetts
  • Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification
  • Three years of clinical experience caring for patients with complex medical diagnoses
Preferred Qualifications
  • Bachelor's degree in Nursing
  • Clinical experience with stroke and cerebrovascular diseases
  • Bilingual

Tufts Medical Center offers a competitive salary range of $85,000 - $110,000 per year, depending on experience. Our comprehensive benefits package includes health, dental, and vision insurance, 403(b) retirement plan, and paid time off.
