As a Home Health Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities. Your expertise will enable patients to regain their independence and live healthier, happier lives.

Key Responsibilities:
  • Plan and administer prescribed skilled physical therapy treatment and training for patients.
  • Test and screen patients' physical strengths to assist physicians in evaluating their level of function.
  • Develop and implement a conditioning and rehabilitation program consistent with the physician's Plan of Treatment.
  • Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program.
  • Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, and supervise and evaluate their performance.
Requirements:
  • Degree from an accredited Physical Therapy Program.
  • Minimum of one year physical therapy experience preferred.
  • Current and unrestricted Physical Therapy license.
  • Current CPR certification.
  • Strong organizational and communication skills.
