Job Description for Skilled Nursing Caregiver


 



  • We are seeking a skilled RN to provide total care for individuals at the end of life at West Texas Rehab Center.
  • The position requires good listening and communication skills, along with the ability to assimilate seemingly unrelated activities into a cohesive whole structure.
  • Organization and planning are necessary, along with the ability to explain plans and goals to others.
  • After hour on call and travel are required.

Key Responsibilities:



  • Provide total care for individuals at the end of life, including skilled nursing care and participation in interdisciplinary care.
  • Utilize good listening and communication skills to build strong relationships with patients, families, and healthcare teams.
  • Develop and implement effective care plans that meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and excellent patient outcomes.

 


Requirements:



  • Current RN licensure
  • Effective communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Basic computer skills and proficiency in electronic medical records
  • After hour on call and travel requirements
