About the Role
We are seeking a skilled Nurse Practitioner (APRN) to join our team. As a Nurse Practitioner at Abode Care Partners, you will be responsible for providing evidence-based medical care to seniors and patients with chronic health issues. Your primary focus will be on performing assessments, collaborating with healthcare professionals, and maintaining up-to-date medical records.
Responsibilities
- Perform comprehensive assessments and develop individualized care plans for patients with complex health needs.
- Provide ongoing medical care, including diagnosing and treating acute and chronic illnesses, injuries, and other health conditions.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Maintain accurate and detailed patient medical records, adhering to HIPAA and OSHA regulations.
Requirements
- A valid state Nurse Practitioners license
- Current National Board Certification as a Nurse Practitioner
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
