Job Title: Community Health Worker

At Chaparral Medical Group, we are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in improving health outcomes for patients with complex medical and social needs.

Key Responsibilities:
  • Manage a caseload of patients, providing education, motivation, and peer support to promote positive behavior changes.
  • Build rapport with patients to increase the likelihood of successful health outcomes.
  • Coach patients to minimize risks associated with common health conditions and behaviors.
  • Accompany patients to medical and social services appointments, ensuring they receive the care they need.
  • Connect patients to local, county, and state services, advocating on their behalf to address barriers to care.
  • Document information from every encounter in designated information systems.
  • Outreach and engage with eligible patients to enroll them in the Akido ECM Program.
Qualifications and Experience:
  • High School Diploma or equivalent.
  • Bilingual in English and Spanish is strongly preferred.
  • 2+ years of experience working with vulnerable populations, including people experiencing homelessness, adults with SMI/SUD, and adults transitioning from incarceration.
  • Ability to work independently and as part of a team.
  • Ability to prioritize multiple tasks and communicate effectively.
  • Maintenance of reliable transportation.
  • Proficiency in operating a computer and documenting information into designated systems.
  • Travel 50-75% - Must have a valid driver's license, automobile insurance, and reliable transportation.
