Job Description:

We're seeking an experienced Advanced Primary Care Provider to join our team of dedicated healthcare professionals. As an integral part of our organization, you'll play a vital role in providing high-quality patient care, developing strong relationships with our patients and families, and contributing to our mission of delivering exceptional healthcare services.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans that address their unique requirements.">
  • Deliver high-quality primary care services, including routine check-ups, screenings, and treatments, while maintaining accurate records and communicating effectively with patients and their families.">
  • Develop and maintain strong relationships with patients, families, and healthcare providers, ensuring seamless coordination of care and optimal outcomes.">
  • Collaborate with interdisciplinary teams to share knowledge, best practices, and resources, enhancing the overall quality of care and patient satisfaction.">
  • Stay up-to-date with the latest medical research, technologies, and evidence-based practices, applying this knowledge to improve patient outcomes and advance our organization's mission.">

Requirements:

  • ">
  • A Master's degree in Nursing or related field, with a current Licensed Family Nurse Practitioner (LFNP) certification in Arizona.">
  • At least three years of experience as a nurse practitioner, preferably in home health, physician's office, or public health settings.">
  • Bilingual skills (English/Spanish) are highly desirable, as well as experience working with diverse populations and age groups.">
  • Excellent communication and interpersonal skills, with the ability to work effectively in fast-paced environments and adapt to changing priorities.">
