Job Description

Inspire health, serve with compassion and be the difference in the lives of our patients and their families. We are seeking a dedicated Hospice Registered Nurse to join our team at Prisma Health.

Responsibilities
  • Patient Assessment: Conduct comprehensive assessments of patients' physiological, psychosocial, developmental, and environmental needs, as well as their families' need for supportive care.
  • Care Planning: Develop individualized nursing plans of care, taking into account patient and family input, to ensure holistic care.
  • Education and Support: Provide education to patients and families on disease management, symptom control, and end-of-life issues, while monitoring their need for additional services.
  • Quality Care: Perform appropriate nursing interventions, document thoroughly in patients' charts, and evaluate the effectiveness of specific interventions.
  • Bereavement Support: Offer bereavement support to families during the grieving process, ensuring their emotional well-being.
  • Continuity of Care: Facilitate seamless communication with other healthcare professionals and agencies, ensuring accurate and timely exchange of information.
Requirements
  • An Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
  • A valid driver's license and auto insurance certificate
  • Minimum 2 years of acute care nursing experience (Preferred: BSN, hospice or home care services experience)
