Therapist Rehabilitation Specialist

VitalCore Health Strategies, an industry leader in healthcare services for correctional facilities, is seeking a highly skilled Physical Therapist to join our team in Hopkinton, MA.

The successful candidate will assist patients with mobility issues due to injury, illness, or disability by creating personalized rehabilitation plans. You will work closely with our medical team, led by the Site Medical Director, to ensure seamless care delivery.

• Document patient encounters in electronic medical records according to SOAP format.
• Collaborate with multidisciplinary teams to develop treatment plans tailored to individual needs.
• Utilize various therapeutic techniques, including exercise therapy, manual therapy, and modalities, to promote patient recovery.
• Communicate effectively with patients, families, and colleagues to ensure optimal care coordination.

Minimum Requirements:

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current licensure to practice as a Physical Therapist in the state of Massachusetts.
• Excellent communication and interpersonal skills.
