BayMark Health Services is seeking a highly skilled Case Manager to join our team in Ohio.

Job Description:

The successful candidate will be responsible for enhancing the overall treatment process by assisting therapists to meet the treatment needs of clients. This includes acting as a liaison between therapists, referring agencies, and family members, arranging family visits, coordinating transportation, and ensuring clients' needs are met.

Key Responsibilities:
  • Act as a liaison between therapist, referral source, and client's family to ensure effective communication and coordination.
  • Arrange family visits, coordinate transportation, and ensure clients' needs are met.
  • Attend court hearings as necessary.
  • Ensure documentation meets policy requirements.
  • Prepare reports for treatment staffing and attend supervisory sessions.
  • Facilitate case management, group sessions, and individual sessions as assigned.
  • Participate in agency quality assurance committees and assist with in-service training.
Requirements:

To be successful in this role, you will need:

  • A Bachelor's degree in Counseling or related field, or equivalent experience.
  • CDCA/LCDC II/LCDC III licensure.
  • Experience working with adolescents, teens, and adults with AOD diagnoses, or relevant work history.
  • A positive, non-judgmental attitude toward patients.
