Job Summary

">As a Registered Nurse Case Manager, you will be responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. You will coordinate efforts within the Rehabilitation team and collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions.

Your Responsibilities

">* Provide program orientation to patients/families/caregivers, including the case manager's role, rehabilitation philosophy, continued stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
* Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status through Family Conferences and other interactions.
* Ensure implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences, facilitating involvement throughout the rehabilitation process.
* Document findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient/family/caregiver.
* Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.

Requirements

">* 1+ years' experience in case management.
* Associate Degree in Nursing or Nursing Diploma; Bachelor's Degree in Nursing preferred.
* Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation; Certification in Case Management preferred.
