Job Summary:

The MDS Nurse (LPN/LVN) plays a vital role in coordinating the Resident Assessment Instrument (RAI) process at Rehab Center of Cheraw. This position is responsible for ensuring the accurate completion of the Minimum Data Set (MDS) and the development of the Interdisciplinary Plan of Care. The successful candidate will work closely with the Director of Nursing (DON) and other members of the healthcare team to provide high-quality care to residents.

Key Responsibilities:

Coordinating the RAI process to ensure accurate and timely completion of the MDS
Developing and implementing the Interdisciplinary Plan of Care
Participating in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM)
Establishing and maintaining positive relationships with all members of the healthcare team

Requirements:

Current, unencumbered LPN/LVN license (as required by state)
One or more years of direct hands-on nursing and MDS/RAP experience preferred
Ability to utilize local and consultant resources in the execution of job responsibilities
Ability to develop an adequate knowledge of state, federal, and regulatory requirements related to the job
