Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Clinical Care Coordinator in our Primary Care department. As a valued member of our healthcare team, you will play a crucial role in providing exceptional patient care and support to our patients and families.

Clinical Responsibilities

As a Clinical Care Coordinator, you will assist practitioners in the performance of procedures, handle prescription orders and refills, maintain accurate patient records, and relay test results. You will provide safe and quality nursing care under the direction of SHS practitioners and the Clinic Supervisor within your scope of practice. Your expertise will be invaluable in ensuring that our patients receive the highest level of care and support.
