Job Description

As a Patient Care Specialist, you will be responsible for providing high-quality patient care and support to our patients and their families.

Your primary focus will be on ensuring the delivery of exceptional patient outcomes through effective assessment, planning, implementation, and evaluation of care.

You will work closely with our interdisciplinary teams to develop and implement comprehensive care plans that address the unique needs of each patient.

Patient Care Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Implement and evaluate care plans to ensure optimal patient outcomes
  • Collaborate with healthcare professionals to provide seamless patient care
  • Document patient information accurately and efficiently

Patient Safety:

  • Administer medications as prescribed by physicians
  • Maintain accurate and timely documentation of patient information
  • Report any safety concerns or incidents promptly

Quality/Customer Service:

  • Participate in quality improvement initiatives to enhance patient care
  • Provide excellent customer service to patients and families
  • Maintain open communication with healthcare teams

Financial Responsibility:

  • Manage time and resources effectively to optimize productivity
  • Meet financial targets and goals

Personal Growth & Competency:

  • Maintain licensure and certification
  • Complete ongoing education and training
  • Participate in professional development opportunities

Qualifications:

  • Bachelor's degree in Nursing or related field
  • Current licensure to practice nursing in the state of employment
  • Minimum 1-2 years of experience in patient care
