Job Summary:

CareStar is seeking a dedicated Intensive Support Coordinator to join our Clinical department. As an Intensive Support Coordinator, you will play a vital role in identifying, coordinating, and overseeing the delivery of services to enhance the health, safety, and well-being of participants. Your primary goal will be to assist participants in achieving maximum independence.

Key Responsibilities:

Identify and coordinate services to meet the unique needs of participants
Develop and implement service plans to enhance health, safety, and well-being
Collaborate with healthcare professionals and other stakeholders to ensure comprehensive care
Provide care management and coordination services to support participants' goals
Maintain accurate records and reports to ensure compliance with regulatory requirements

Requirements:

Bachelor's degree in a human services field, such as Social Work, Psychology, or Sociology
Two years of direct service experience serving individuals with disabilities or related disabilities
Two years of experience in healthcare, habilitative, or rehabilitative settings
Effective organizational and communication skills
Ability to work flexible schedules and travel as needed
