Your Role:

As a Hospice Nurse Specialist, you will be responsible for providing high-quality patient care and support to individuals with life-limiting illnesses.

Key Responsibilities:

  • Patient Assessment: Conduct thorough assessments of new admissions to develop personalized care plans.
  • Care Coordination: Collaborate with healthcare teams to ensure seamless continuity of care.
  • Communication: Regularly update clinical managers, physicians, and care teams on patient progress.
  • Care Documentation: Maintain accurate and timely records of patient care.
  • On-Call Services: Provide emergency support to patients and families as needed.

Requirements:

  • RN License: Current licensure specific to the state(s) assigned.
  • Nursing Experience: One+ year of experience required.
  • CPR Certification: Current certification.
  • Driver's License: Valid driver's license and reliable transportation.
