Care Coordination Specialist

We are seeking a dedicated Care Coordination Specialist to join our team in this mission. This role will play a critical part in ensuring seamless transitions of care for complex renal patients, improving their health outcomes and quality of life.

As a Care Coordination Specialist, you will be responsible for conducting comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs, identifying gaps in care and barriers to good health. You will then work closely with patients, their healthcare providers, and other members of the care team to create and implement personalized care plans that address these needs and improve patient outcomes.

Key Responsibilities:

  • Conduct thorough transition of care assessments, including identifying medical, behavioral, pharmaceutical, and social needs of patients
  • Create and implement care plans to address identified needs and improve patient health
  • Coordinate care by serving as an advocate and resource for patients, their families, and healthcare providers
  • Facilitate care across various settings, including the home, hospital, skilled nursing facility, and acute care facility
  • Manage patient care during periods of transition to ensure effective transitions and minimize avoidable readmissions

Requirements:

  • Bachelor's degree in nursing or relevant field
  • 3-5 years of experience in case management or care coordination, preferably in coordinating care across multiple settings
  • Current, unrestricted compact Registered Nurse license
  • Valid BLS certification from a licensed AHA or American Red Cross training facility or provider

Salary: $65,000 - $80,000 per year, depending on location and experience.
