About the Role

We are seeking a skilled Clinical Case Manager to join our team and provide essential support services to adults with mental health needs. This is an excellent opportunity to make a meaningful difference in the lives of individuals and their families.

Job Summary

The Clinical Case Manager will work closely with clients, healthcare providers, and community organizations to develop and implement personalized care plans. This role requires strong communication, interpersonal, and problem-solving skills to ensure effective service delivery and positive outcomes.

Key Responsibilities
  • Evaluate client needs and develop comprehensive care plans that address physical and emotional well-being.
  • Collaborate with clients, family members, and healthcare professionals to ensure seamless coordination of services.
  • Link clients to community resources, such as housing, employment, and education services, to promote independence and self-sufficiency.
  • Monitor client progress, adjust care plans as needed, and document all interactions and interventions.
  • Participate in multidisciplinary teams to share best practices, discuss client challenges, and explore innovative solutions.
Requirements
  • Bachelor's degree in social work, psychology, or a related field; or equivalent experience in mental health or a related field.
  • CDCA certification required for SUD (Substance Use Disorder) treatment.
  • Valid driver's license and reliable transportation to travel to client homes and community sites.
  • Excellent communication, interpersonal, and problem-solving skills to work effectively with diverse populations.

Compensation and Benefits

The estimated salary range for this position is between $45,000 and $55,000 per year, depending on experience and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package, including medical, dental, vision, and life insurance, as well as opportunities for professional growth and development.
