Maternity Navigator, Lawrence Ob/Gyn Specialists, FT Days

We are seeking a highly skilled and compassionate RN Maternity Navigator to join our team at LMH Health. As a Maternity Navigator, you will play a vital role in ensuring the best possible outcomes for our patients and their families.
Key Responsibilities- Serve as a liaison between patients, providers, and community referral agencies to ensure optimal health outcomes and achieve quality measures.
- Perform needs-based assessments of clients and provide accurate, consistent information to inform care decisions.
- Collect and update demographic information, ensuring complete and accurate documentation in our computerized database.
- Collaborate with social workers, healthcare providers, and directors as needed to ensure seamless care coordination.
- Provide non-judgmental advice based on protocols and resources, maintaining a patient-centric, customer-driven professional attitude.
- Function independently, demonstrating leadership in decision-making, communication, and problem-solving techniques to resolve clients' questions and concerns.
- Current Registered Nurse license to practice in Kansas.
- Current Basic Life Support for Healthcare Providers (BLS).
- Minimum 3 years of RN experience in an acute setting.
