Key Responsibilities
  • Conduct thorough evaluations and assessments of patients to identify their physical therapy needs
  • Develop and implement individualized treatment plans to address patient goals and objectives
  • Provide skilled interventions, including therapeutic exercises, neuromuscular re-education, and gait training
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Bachelor's degree in Physical Therapy from an accredited program
  • Current licensure as a Physical Therapist in the state of practice
  • Minimum of one year of experience as a Physical Therapist in a home health setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
