Kaiser Permanente seeks a skilled Home Health Physical Therapist for a position in San Jose, California.
Job OverviewWe are committed to providing high-quality care to our patients. As a Home Health Physical Therapist, you will play a critical role in delivering exceptional patient outcomes.
About the RoleThis is an ongoing opportunity to work as a Physical Therapist making home visits or working in patients' residences. In accordance with agency policy and state/federal regulations, you will provide independent physical therapy to patients in their place of residence. You will teach patients/caregivers and educate on preventative and rehabilitative procedures. Develop and coordinate the plan of care, providing case management as needed.
- Evaluate patients, interpreting findings to determine dysfunction and adaptive equipment needs.
- Establish treatment goals and plans treatment to achieve established goals.
- Initiate discharge planning for physical therapy, implementing the plan through direct treatment and coordinating with other healthcare personnel.
- Assess patients for adaptive equipment, based on clinical guidelines, and coordinate assessment with vendors as necessary.
- Reassess adaptive equipment as needed, discharging patients from physical therapy when maximum benefit is achieved.
- Participate in educational and clinical research activities in physical therapy and participate in specialized patient care clinics outside the department.
- Supervise Home Health Aides according to regulation and agency policy, demonstrating progress toward meeting professional practice criteria.
- Provide accurate documentation of physical therapy patient management, including assessment for adaptive equipment.
To be successful in this role, you must have:
- A minimum of two (2) years of experience in physical therapy.
- A graduate degree in physical therapy from an accredited curriculum.
- A valid driver's license, Physical Therapist License (California), and National Provider Identifier.
- Basic Life Support certification.
- Knowledge of Medicare conditions of participation and OASIS.
- Clinical skills to detect early warning signs of complications, including medical assessments of skin integrity and mentation.
- Able to differentiate emergent versus non-emergent patient care status.
- A commitment to Service Orientation.
- Effective English written and oral communication skills.
- Good interpersonal skills.
The estimated annual salary for this role is $85,000 - $110,000, depending on experience and qualifications.
