Care Coordinator Specialist

We are seeking a Care Coordinator Specialist to join our team at St. Luke's Health System in Nampa, Idaho.

Job Summary:
This is a full-time position that offers a competitive salary range of $65,000 - $85,000 per year, depending on experience. The successful candidate will have a Master's degree in social work and a variable schedule that includes days, nights, and weekends. We offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as a 403b retirement plan and employee assistance programs.
Responsibilities:
- Provide intensive onboarding and training to new staff members
- Develop and implement strategies to improve patient outcomes and satisfaction
- Collaborate with healthcare teams to ensure seamless care transitions
Requirements:
- Masters degree in social work
- 2+ years of experience in a healthcare setting
- Excellent communication and interpersonal skills
