As a Clinical Care Specialist, you will play a vital role in our team of exceptional physicians, nurses, and staff. Your primary responsibility will be to provide acute patient care services, ensuring seamless transitions across the continuum of care.

Key Responsibilities:
  • Conduct thorough histories and physical examinations to inform comprehensive patient assessments.
  • Participate in daily rounds with physicians to discuss patient care plans and facilitate timely interventions.
  • Collaborate with clinical staff to develop and implement individualized treatment plans, including ordering diagnostic studies and therapies as necessary.
  • Initiate and monitor treatments, such as device placement, physical therapy, and home health referrals, to optimize patient outcomes.
  • Demonstrate expertise in providing care appropriate to diverse patient populations, adhering to evidence-based guidelines and best practices.
  • Perform procedures as indicated for diagnosis and management of patient conditions, ensuring accurate documentation and communication with interdisciplinary teams.
Requirements:
  • Graduate of an accredited physician assistant program or master's degree in advanced clinical practice, approved by the National Commission on Certification of Physician Assistants (NCCPA).
  • One year of experience working in primary care management or acute care settings, preferred.
  • NJ Physician Assistant License or Advanced Practice Nurse – NJ License, as applicable.
