Compensation: The estimated hourly pay range for this position is $38-$58 per hour.

About the Role

This Advanced Care Professional opportunity offers the chance to make a real difference in the lives of others while enjoying a flexible schedule that fits your needs. You will work closely with our team to provide compassionate nursing care to our residents in post-acute facilities.

  • Monitor patients' physical and mental conditions and track their vital signs
  • Perform resident intakes, documenting their medical histories and any health concerns
  • Help with the administration and proper verification of medications, tracking their use and effects
  • Cultivate a comfortable, safe facility for residents
  • Track residents' medical records and charts
  • Assist with the set up of IVs, immunizations, and phlebotomy procedures
  • Keep track of medical supplies and maintain a proper inventory
Qualifications and Skills

To be successful in this role, you will need to have a current Connecticut state license to practice as a Licensed Practical Nurse (LPN), completion of an approved LPN educational program, and a minimum of 1 year of experience (LPN or related role). We also require a high school diploma or GED (or equivalent).
