Job Summary

We are seeking a skilled Hospice RN Case Manager to join our team. As a key member of our interdisciplinary group, you will provide direct patient care services to hospice patients and supportive care to their families.


The ideal candidate will have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology. A current driver's license and automobile insurance are also required.


Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to their families.
  • Maintain primary control and professional management of each patient.
  • Act as the primary liaison between physicians, patients/families, and the hospice team.
