The salary for this role is $88,000 to $173,200 annually, depending on location and experience. UnitedHealth Group offers a comprehensive benefits package, including incentive and recognition programs, equity stock purchase, and 401k contribution.

Job Responsibilities:

  • Conduct in-home assessments to evaluate patients' medical history, review systems, reconcile medications, take vital signs, and perform comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing when necessary.
  • Identify diagnoses to inform care management and active medical management in furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment.
  • Communicate assessment findings to inform members' primary care physicians of potential gaps in care.
  • Educate members on disease processes, medication adherence, and compliance.
  • Recognize and address urgent and emergent situations.

Requirements:

  • Active NP or PA license or ability to obtain one by start date. Licensure must be unencumbered, without any unresolved disciplinary actions.
  • Active national NP or PA certification or ability to obtain it by start date. National certification through ANCC or AANP (for NPs) or NCCPA (for PAs).
  • Ability to complete job requirements, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visits.
