Job Description

We are seeking a skilled Dialysis Nurse - Clinical Excellence Specialist to join our team. As a key member of our Home Therapy program, you will be responsible for providing quality care to patients with end-stage renal disease (ESRD). Your expertise will play a crucial role in delivering exceptional patient outcomes and maintaining our position as an industry leader in home dialysis.

Key Responsibilities
  • Provide comprehensive training and ongoing support to patients choosing a home dialysis modality, ensuring they understand their treatment options and can effectively manage their condition.
  • Conduct regular home visits to assess the patient's environment and provide education on infection control and emergency preparedness procedures.
  • Document all nursing services accurately and thoroughly in the Electronic Medical Record, ensuring continuity of care and facilitating communication among healthcare providers.
  • Promote clinical excellence by continually seeking opportunities to improve patient and quality outcomes through tracking and trending of program and patient performance.
  • Collaborate with the technical services department to ensure proper equipment operation and perform routine and non-routine laboratory tests as ordered.
Requirements and Qualifications
  • Certified Registered Nurse (CRN) with experience in hemodialysis or peritoneal dialysis.
  • Strong knowledge of federal and state regulations governing home dialysis programs and patient care requirements.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Familiarity with electronic medical records and documentation software.
Compensation and Benefits

The estimated salary for this position is $75,000 - $90,000 per year, depending on experience. Our total rewards package includes:
