In this role, you will have the opportunity to make a positive impact on the lives of patients and families in our community. As a Clinical Therapist - Chronic Illness Specialist at Avera Medical Group Family Medicine/Avera Now Mitchell, you will be responsible for assessing the needs of patients facing chronic illness and facilitating plans with patients and families to promote effective care.

With a focus on teamwork and compassionate care, this position requires a Master's Degree in Social Work and relevant healthcare experience. You will work closely with a multidisciplinary team to provide comprehensive patient care, addressing emotional and social needs while facilitating effective care plans.

Key Responsibilities:
  • Assess the needs of patients facing chronic illness and develop individualized care plans.
  • Facilitate communication among patients, families, and healthcare providers to ensure comprehensive care.
  • Maintain a current knowledge base of available community resources and referrals.
  • Provide direct clinical therapeutic services to patients as part of an overall comprehensive patient care plan.
Requirements:
  • Master's Degree in Social Work required from a program accredited by the Council on Social Work Education.
  • 1-2 years' healthcare social work experience required; 3-5 years' experience preferred.
  • Certified Social Worker (CSW-PIP) License in the State of South Dakota required; multi-state licensure may be required.
