At-Home Physical Therapist Position

We are seeking a compassionate and motivated individual to join our team as an At-Home Physical Therapist at JAG Physical Therapy. In this role, you will work closely with patients in their home environment to enhance their functional abilities and overall quality of life.

Job Description

You will collect and record patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers. This includes initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.

You will prepare for evaluations according to department policies/procedures and standards of practice, obtaining patient history, exhibiting knowledge of patient diagnoses, and researching unfamiliar diagnoses.

You will select and administer tests and measures, synthesizing evaluation results to formulate the diagnosis, goals, prognosis, and plan of care.

You will establish treatment plans based on evaluation results, goals, and best practice, collaborating with interdisciplinary teams, patients, and patient support systems.

You will assess and re-assess the need for modifications to the plan of care and goals, identifying the need for consultations and/or referring patients to other healthcare professionals or services.

You will initiate and complete discharge plans, providing education/training to patients and/or patient support systems.

You will be responsible for treatment, plan of care, exercise prescription for your patient caseload, providing interventions to patients that are safe, effective, and in compliance with JAG Physical Therapy.

Requirements
  • Bachelor's or Master's Degree required, Doctorate degree preferred
  • Licensed Physical Therapist in the state practicing in
  • Completed state-regulated CEU requirement in the appropriate state (NY, NJ)
  • Valid CPR License
  • Experience/knowledge with an EMR software is preferred
  • Excellent oral, written communication
  • Strong clinical decision-making skills
  • Excellent work ethic and dedication to patient success
  • Basic computer skills
  • Works as a team member and individually with minimal supervision
  • Maintains a positive, professional demeanor at all times
