Telephonic RN Case Manager Opportunity

We have an exciting opportunity for a Telephonic RN Case Manager to join our team in Richardson, TX. As a key member of our team, you will play a critical role in making a solid connection between exceptional patient care and outstanding career opportunities. In this hybrid role, you will work from home 2 days a week and in the office for the remaining three days, supporting a diverse member population with education, advocacy, and connections to the resources they need to feel better and get well.
With a strong focus on clinical operations and medical management activities across the continuum of care, you will identify, coordinate, or provide appropriate levels of care under the direct supervision of an RN or MD. This includes case management, coordination of care, and medical management consulting, as well as providing health education, coaching, and treatment decision support for members.
As a highly motivated and organized individual, you will be responsible for making outbound calls and receiving inbound calls to assess members' current health status, making referrals to outside sources, identifying gaps or barriers in treatment plans, and providing patient education to assist with self-management. You will also support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels.
This is a high-volume, customer service environment that requires efficiency, productivity, and thoroughness when dealing with our members over the phone. Solid computer and software navigation skills are essential, as is being patient-focused and adaptable to changes. The estimated salary range for this position is $70,000 - $110,000 per year, depending on experience. If you are located in Richardson, TX, you will have the flexibility to work from home 2 days a week and in the office for the remaining three days in this hybrid role. Work schedule is Monday-Friday, business hours for three days, then 11am-8pm for two days a week. This role offers a comprehensive benefits package, including medical, dental, vision, 401(k), and paid time off.
To be successful in this role, you will require:
- An undergraduate degree or equivalent experience
- A current, unrestricted RN license in the state of employment
- 3+ years of experience in a hospital, acute care, or direct care setting
- The ability to support a Hybrid work schedule, including consumer-centric hours (11a-8p CST) two days a week
- The ability to type and navigate a Windows-based environment
