Healthcare Navigator

We are seeking an experienced RN or LPN with a passion for patient-centered care to join our team as a Transition Care Navigator. In this role, you will work closely with our patients, families, and healthcare teams to provide comprehensive care planning, coordination, and education.

As a Transition Care Navigator, you will be responsible for assessing patient needs, developing personalized care plans, and facilitating communication between healthcare providers. Your expertise and empathy will make a meaningful difference in the lives of our patients and their loved ones.

• RN or LPN licensure in good standing
• Experience in home health or transitional care
• Excellent communication and interpersonal skills

We offer a competitive salary range of $57,000 to $90,000 per year, based on experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, paid time off, and professional development opportunities.
