About the Job

We are seeking a skilled Nurse Case Manager to join our team. This role involves working closely with physicians, registered nurses, and other healthcare professionals to ensure that patients receive the best possible care.

As a Nurse Case Manager, you will be responsible for conducting initial reviews of patient information, participating in daily multidisciplinary rounds, and developing discharge plans to meet the needs of post-acute care services.

You will also communicate with patients, their families, outside vendors, post-acute care providers, and teammates to ensure seamless transitions of care.

Responsibilities

  • Conduct initial reviews of each patient's information and initiate contact
  • Participate in daily multidisciplinary rounds (MDR) and weekly length of stay (LOS) meetings to discuss and review patient care
  • Take a collaborative care approach that assesses clinical, spiritual, financial, and educational baselines to work towards realistic results
  • Develop and implement discharge plans for patients to meet their needs for post-acute care services, avoid outlier days, reduce the length of stay (LOS), and reduce readmission rates
  • Communicate with patients, their families, outside vendors, post-acute care providers, and teammates

Salary Information

The estimated salary for this role is $35.47 - $46.63 per hour, based on industry standards and location.

Qualifications

  • Unencumbered license to practice in the State of Arkansas
  • 3 years of acute care nursing experience required
  • BSN preferred, or Associate Degree with CCM or ACM Certification within 2 years of hire date
  • Previous case management, utilization management, or discharge planning experience strongly preferred
