Job Title: Physical Therapist Assistant - New Hampshire Therapy

About the Role:

We are seeking a skilled and compassionate Physical Therapist Assistant to join our team in New Hampshire. As a Physical Therapist Assistant, you will work closely with our interdisciplinary team to provide high-quality physical therapy services to patients in their homes.

Responsibilities:

  • Collaborate with the interdisciplinary team to develop patient-centered plans of care focused on achieving physical therapy goals.
  • Instruct, coach, and guide patients and families to support them in achieving optimal health and well-being.
  • Promote optimal health, well-being, and quality of life for home health patients and their families through skilled, intermittent physical therapy.
  • Practice independently in patients' homes using evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.
  • Act as case manager and team lead for patients requiring home health therapy services when nursing is not involved or physical therapy is the primary patient need.
  • Assess patients and collaborate with the interdisciplinary team and primary care provider to develop patient-centered plans of care.
  • Provide physical therapy treatment to patients and families as ordered.
  • Complete accurate clinical documentation to effectively support services provided within specified timeframes.
  • Provide physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth and development and life stages.
  • Maintain communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
  • Report acute medical problems and condition changes to the primary care provider.
  • Provide supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.
  • Complete regulatory required assessments within specified timeframes.
  • Complete medication reconciliation at specified timeframes.

Requirements:

  • 1-2 years of experience in an outpatient rehabilitation setting.
  • At least two years of physical therapy experience highly preferred.
  • One year of experience in home health or other health setting with geriatric emphasis.
  • Current license in Physical Therapy or ability to obtain prior to hire.
  • American Heart Association HealthCare Provider BLS preferred upon hire and required within 90 days of hire.
  • Current Driver's license.
  • Excellent customer service skills.
  • Ability to work without direct supervision.
  • Provides care appropriate to the age of patients served on the job.
  • Knowledge of physical therapy assessment and treatment methods.
  • Knowledge of physical therapy modalities and equipment.
  • Ability to maintain effective relationships with other healthcare professionals.
  • Must be able to develop skills in use of technology used in home care settings.
  • Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.

Compensation: $65,000 - $80,000 per year, depending on experience and qualifications.
