About the Role

We are seeking a highly skilled Long Term Care Nurse Specialist to join our team at Caraday Healthcare. In this role, you will play a vital part in ensuring that our residents receive the highest level of care possible.

Job Summary

The Long Term Care Nurse Specialist will assist the Director of Nursing in ensuring that documentation meets federal, state, and certification guidelines. Key responsibilities include coordinating the RAI process, ensuring timeliness and completeness of MDS, CAAS, and Interdisciplinary Care Plan.

Responsibilities
  • Assist in adhering to federal and state regulations and certification requirements
  • Participate in regulatory or certification survey processes and correct deficiencies
  • Report trends from completed audits to the Quality Assurance Committee
  • Ensure completion of the RAI Process from MDS through completion of the plan of care
  • Initiate and monitor RAI process tracking, discharge/re-entry, and Medicaid tracking forms through PCC & SimpleLTC systems
  • Follow up with staff to ensure compliance to standards of documentation
  • Collect data for each resident and interview staff and residents as necessary to ensure good standard of practice
  • Facilitate accurate determination of the Assessment Reference Date that accurately reflects patient care needs
  • Provide interdisciplinary schedules for all MDS assessments and care plans as required by OBRA and PPS
  • Obtain required signatures
  • Coordinate scheduling notice of patient care planning conferences and communicate outcomes/problems to responsible staff, patients, and/or responsible parties
Requirements

To be successful in this role, you will need:

  • A current RN license
  • CPR certification (30 days provided to complete a course and get certified)
  • Excellent knowledge of RAI Process, Medicare PPS process, and Medicaid reimbursement
  • Thorough understanding of the Quality Indicator process
  • Knowledge of OBRA regulations and Minimum Data Set
Estimated Salary

$60,000 - $80,000 per annum, depending on experience.
