We are seeking a highly skilled Wound Care Specialist to join our team at HRMD/Texas Pain Physicians. As a key member of our healthcare staff, you will be responsible for providing exceptional care to patients in their homes.

Job Summary:

The Wound Care Specialist will be responsible for assessing, treating, and managing wounds for patients in their homes. This role involves developing personalized care plans, educating patients and families, and coordinating with other healthcare professionals to optimize wound healing and overall patient health.

Responsibilities:

  • Patient Assessment and Care: Conduct thorough assessments of patients' wounds, including evaluating size, depth, and any signs of infection or complications. Develop and implement individualized wound care plans based on assessment findings and best practices. Perform wound care procedures, including cleaning, debridement, dressing changes, and applying topical treatments as needed.
  • Monitoring and Documentation: Regularly monitor and document wound progress, treatment efficacy, and any changes in the patient's condition. Record detailed notes on patient interactions, care provided, and patient responses to treatments. Adjust care plans as necessary based on ongoing assessments and patient feedback.
  • Patient and Family Education: Educate patients and their families on wound care techniques, infection prevention, and overall management of the patient's condition. Provide guidance on how to perform at-home wound care tasks and address any questions or concerns.
  • Coordination and Communication: Coordinate with other healthcare providers, such as physicians, therapists, and home health aides, to ensure comprehensive patient care. Communicate effectively with patients, their families, and the healthcare team regarding care plans, progress, and any issues that arise.
  • Infection Control and Safety: Follow strict infection control practices to prevent wound infections and maintain a safe home environment. Manage and oversee the use of wound care supplies, ensuring proper handling, storage, and disposal.
  • Quality Improvement: Participate in quality improvement initiatives related to home wound care. Stay current with the latest advancements in wound care techniques and technologies through continuing education and professional development.

Qualifications:

  • Education: Bachelor's degree in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited institution.
  • Licensure: Current Registered Nurse (RN) license in the state of practice.
  • Experience: Prior experience in wound care and/or home health nursing is preferred.

Salary: $40-$50 per hour, depending on experience

Working Conditions: The Wound Care Specialist will work in patients' homes, which may vary in terms of size, location, and accessibility. Travel between multiple patient homes within a designated area may be required. Physical stamina is necessary for activities such as lifting and moving patients, as well as standing for extended periods. Exposure to potentially hazardous materials, including infectious agents, is possible.
