Nurse-Family Partnership (NFP) Community Health Worker

The NFP Community Health Worker plays a vital role in fostering long-term success for first-time parents, their babies, and addressing systemic problems affecting our community. This position will report to the Manager, Nurse-Family Partnership Community Health Workers.

This position will assist Indiana NFP teams, particularly serving rural areas, with directing their participating families to quality resources in their communities based on each family's specific needs, strengths, and motivation as identified by their Nurse Home Visitor(s) and the Manager, Nurse-Family Partnership Community Health Workers.

Key Responsibilities:

  • Build relationships with NFP staff, NFP families, and networks with key community partners to enhance the menu of wrap-around services provided.
  • Create easily accessible rapid-response and long-term solutions for NFP nurses and families to connect effectively to quality community resource networks.
  • Ensure timely and accurate entry of required information into NFP databases and files.
  • Participate as needed in NFP CHW team meetings.
  • Participate in continuous innovation projects that improve operational efficiency and evidence-based outcomes.

Required Competencies:

  • Completion (or pending certification) of Certified Community Health Worker (CCHW).
  • Client Engagement - Takes a strengths-based, client-centered approach to developing relationships at all levels while establishing appropriate boundaries.
  • Established Contacts - Maintains and grows a professional network of community resources, health care providers, and others as appropriate to the organization.
  • Communication Skills - Articulates thoughts and expresses ideas effectively using oral, written, visual, and non-verbal communication skills, as well as listening skills to comprehend customers' needs.
  • Technical Knowledge - Has working knowledge of Google Suite.
  • Outcome-Oriented - Meets or exceeds specific goals as quickly as possible.
  • Self- and Team-Reflection - Evaluates one's own attitudes and actions to recognize successes and areas for self-improvement.

Preferred Competencies:

  • Geographical Proficiency - Has extensive knowledge of Indiana's rural communities.
  • Diversity Awareness - Works effectively delivering assistance that is mindful of different cultural/language traditions, showing respect and validation for diverse communities/organizations.
  • Information Monitoring/Reporting - Collects and reviews information to manage or improve processes and/or activities.

Other Requirements:

  • Ability to occasionally work a flexible schedule beyond normal business hours.
  • Willingness to travel to participants' homes, workplaces, community centers, and other venues at times that are convenient to individuals being served.
  • Background Screening - All candidates must pass a pre-employment background check.
