Job SummaryWe're seeking a highly skilled Primary Care Physician Assistant or Nurse Practitioner to join our growing outpatient practices in Aurora, Colorado. This role offers an excellent opportunity to work closely with our medical director and contribute to both care delivery and practice innovation.
As a valued member of our team, you'll have the chance to rediscover the joy of practice while working in a value-based care environment with a supportive team. You'll be empowered to deliver high-quality patient care, partner with our care team, and leverage our coaching and integrated behavioral health models to drive positive change.
Key Responsibilities:
- Provide comprehensive primary care in an outpatient setting.
- Partner with the care team to engage our geriatric patient population in care and drive behavior change.
- Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population.
To be successful in this role, you'll need:
- At least 2 years of primary care experience.
- Current licensure in the state of Colorado (or ability to obtain).
- The ability to build strong relationships with team members and communicate effectively both one-on-one and in groups.
- A commitment to data-driven decision making and a passion for using technology to improve patient outcomes.
- Board Certification.
