We are seeking a compassionate and skilled Medical Social Worker to join our team at LHC Group. As a Medical Social Worker, you will play a vital role in providing medical social services to patients and their families, helping them navigate the complexities of hospice care.

The successful candidate will be responsible for:

• Assessing patients' social and emotional needs and developing individualized plans to address these needs.
• Collaborating with the interdisciplinary team to develop and implement comprehensive care plans.
• Providing education and support to patients and their families on hospice care and related services.
• Coordinating with community resources to ensure patients receive the necessary support and services.

To be successful in this role, you will need:

• A Master's degree in Social Work from a Council on Social Work Education-accredited program.
• At least one year of experience in a healthcare setting, preferably in hospice care.
• Current CPR certification and a valid driver's license.
