Hospice Care Professional

**Key Responsibilities:**
Provide compassionate care to patients in their homes or in our facilities
Work collaboratively with our interdisciplinary team to develop and implement personalized care plans
Maintain accurate and timely documentation of patient care

**Requirements:**
High school diploma or equivalent
Current CNA license in the state of Texas
Valid CPR certification
