Nutrition that Works, LLC is seeking a consultant dietitian to provide clinical nutrition services to our Long Term Care facility in Spencer, WV. The ideal candidate will have excellent communication skills and be credentialed by the CDR.
Key Responsibilities:
- Completing clinical nutrition assessments and documenting using the Nutrition Care Process in the EMR system
- Collaborating with the interdisciplinary care plan team to develop effective care plans
- Completing the MDS - Section K and calculating TF
- Making recommendations for wounds and significant weight changes
Requirements:
- Credentialed by the CDR
- Dietetic licensure
- Positive attitude and client-centered approach
- Good communication skills
