DaVita Licensed Practical Nurse Role

DaVita is seeking a compassionate Licensed Practical Nurse to join our team in an outpatient dialysis center. About the Role

As a Licensed Practical Nurse at DaVita, you will be responsible for delivering high-quality patient care in a fast-paced environment. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans, ensuring the best possible outcomes for our patients.

Key Responsibilities

  • Provide direct patient care, including administering medications, monitoring vital signs, and performing laboratory tests.
  • Develop and implement individualized treatment plans in collaboration with the healthcare team.
  • Conduct patient assessments, identify potential health risks, and develop strategies to mitigate them.
  • Collaborate with the healthcare team to ensure seamless transitions of care and optimal patient outcomes.

Requirements

  • Current LPN-LVN license in the state of practice required.
  • Current CPR certification required.
  • IV certification required if required by state regulations.
  • High school diploma or equivalent required.
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system.
  • BONENT or CHT certification preferred or as required by state regulation.
  • Minimum of one year's experience in a hospital or clinical setting preferred.
  • Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required.
