">The Role
">We are seeking a skilled Palliative Care Specialist to join our team. As a key member of our palliative care team, you will be responsible for ensuring comprehensive, individualized treatment plans for our patients, demonstrating exceptional clinical skills and ethical judgment.
">You will work closely with our Palliative Medical Director and other interdisciplinary team members to deliver high-quality palliative care services, including diagnosis, treatment, and follow-up care for patients referred to our program.
">Your key responsibilities will include:

Conducting thorough assessments of patient needs, identifying disease-specific changes, decline, or changes in function, inadequately controlled pain or other distressing symptoms, falls, infections, or similar events.
">Developing and implementing individualized palliative care plans in consultation with patients, families, and other team members.
">Facilitating access to necessary care and services for patients when needed, according to their plan of care.
">Participating in evening/weekend call as required, providing clinically competent and responsive care to patients.
">Prescribing medications, including controlled substances, within the scope of your licensure and delegation, in accordance with state law.

To succeed in this role, you will need:

A minimum of two years of experience as a Nurse Practitioner, preferably with a specialization in Family or Adult Medicine.
">Current licensure as a Registered Nurse or equivalent, as well as current Nurse Practitioner certification/license.
">A collaborative agreement with a supervising physician, agreeable to supervise work in palliative care, or an agreement to enter such an arrangement with our Palliative Medical Director, as required by state law.
">Part B Provider certification and a Federal DEA license.
