Position Details:
During a visit that may last up to one hour, Matrix providers assess and evaluate a member's current health status, medical history, medication compliance, social circumstances, and other potential risks.
This process offers unparalleled insight into a member's overall health and wellness that can be challenging to capture during standard office visits.
The Matrix Comprehensive Health Assessment enhances care quality and allows us to address multiple care gaps in a single visit.

Key Responsibilities:
What to Anticipate as a Matrix Provider:
Conduct assessments for adults and geriatrics, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in accordance with Matrix protocols.
Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and relevant ancillary medical personnel as necessary.
Engage with Primary Care Physicians (PCP) on patient education and provide follow-up care.
Deliver services across various settings, including home visits, skilled nursing facility visits, and telehealth consultations.
Nurse Practitioner - Flexible Schedule - Home-Based Health Assessments
Employment Type:
PRN/Part Time - Flexible Schedule for Nurse Practitioners
Compensation:
$90.00 or $95.00 per completed assessment, depending on location.
Work Environment:
Home, Skilled Nursing Facilities, Telehealth, and other community settings.
Work Hours:
Flexible scheduling available, including days, evenings, nights, and weekends.
Benefits:
PRN/Part Time providers are non-benefited, except for 401(k) and limited voluntary benefits.
Mileage reimbursement plan available.
Qualifications:
Essential Requirements:
Master's Degree or equivalent experience with successful completion of NP licensure.
Current RN and NP licensure in the state of practice, including prescriptive authority or the ability to obtain it.
Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
Current BLS, ACLS, or CPR certification.
A minimum of one year of experience as a Nurse Practitioner.
Proficient computer skills and familiarity with electronic health/medical record systems.
Excellent verbal and written communication skills with patients, clients, and colleagues.
Ability to adapt to frequent changes with ease.
Travel Requirements:
Must possess a valid state driver's license and have the ability to travel, including proof of adequate automobile insurance coverage.
Travel is necessary to meet patients in their living environments.
