Medical Social Worker

CenterWell Home Health is seeking a Medical Social Worker to join our team. As a Medical Social Worker, you will play a critical role in providing comprehensive care to patients in their homes.
Key Responsibilities- Evaluate and develop personalized plans of care to meet the emotional and social needs of patients
- Assess patients' social and emotional states and provide appropriate interventions
- Collaborate with healthcare teams to develop and implement care plans
- Provide education and support to patients and families
- Supervise and evaluate the performance of Social Work Assistants
- Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Social Worker licensure in the state of practice, if required by state law or regulation
- Valid driver's license, auto insurance, and reliable transportation
- Proof of current CPR certification
- Minimum of one year of experience as a social worker in a healthcare setting
- Competitive pay range: $47,700 - $65,600 per year
