Job Summary

We are seeking a highly skilled Recovery Coach/Case Manager to join our team at La Frontera Empact. The successful candidate will provide case management and coordination services to individuals and families, ensuring quality service delivery and adherence to the Arizona Practice Model and 12 Principles.

Key Responsibilities
  • Provide direct in-home, in-office, and community services, including crisis intervention, transportation, and education.
  • Facilitate and schedule Child and Family Team Meetings/Adult Recovery Team Meetings, completing written documentation and providing copies to team members.
  • Engage and invite natural supports as identified by the client, developing and maintaining effective working relationships with other agency stakeholders.
  • Make referrals to collaborating partners and other community services as client needs indicate.
  • Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact, meeting productivity standards established at a minimum of once a year and as needed due to contract changes.
Requirements
  • Bachelor's degree in a behavioral health-related field or any combination of education/experience that meets BHT requirements as stipulated by AZDHS/BHS.
  • Flexible work hours and ability to meet with individuals in their home, community, or office.
  • Fingerprint clearance card or eligibility to obtain one.
  • Valid AZ driver's license with no major infractions in the past three years.
  • Regular access to reliable transportation.
  • Bi-lingual (Spanish) ability preferred, with Spanish language differential available.
  • At least 21 years of age at time of employment.
