Palliative Care RN

We are seeking a skilled Palliative Care RN to join our Advanced Illness Management team at Illumia Health. As a key member of our clinical team, you will provide essential support and comfort to patients with advanced illnesses and their families.
This role involves triaging patient needs, coordinating care, and collaborating with healthcare teams to enhance patient quality of life while operating afterhours. The successful candidate will ensure timely and effective communication regarding patient conditions and adhere to the highest standards of compassionate care.
As a Palliative Care RN in our Advanced Illness Management Program, you will work closely with our interdisciplinary team to identify patient/family needs and provide preventive and supportive care. You will utilize assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life.
Responsibilities- Triage patient needs over the phone until a nurse can provide an in-person visit
- Respond appropriately and with urgency to crisis calls from patients and families after hours
- Make the initial nursing evaluation in determining eligibility for palliative services, as part of a patient visit, within 48 hours of referral
- Identify the patient/family's physical, psychosocial, emotional, and environmental needs and re-assess as needed
- Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care
- Timely document the problems, appropriate goals, interventions, and patient/family response to palliative care
- Maintain accurate and relevant clinical notes regarding the patient's condition
You will be responsible for effectively communicating with members of the AIM team, clinical supervisor, and the patient's PCP regarding changes in patient status and/or patient needs occurring during afterhours calls.
About YouTo be successful in this role, you will need:
- 2 years of prior skilled Registered Nurse experience
- Demonstrated ability to provide quality patient care
- Nursing experience in palliative care, oncology, hospice, or geriatric care is preferred
