Job Summary

We are seeking a highly motivated and organized individual to join our team as a Care Manager. In this role, you will be responsible for working with individuals and their families to identify needs, develop desired outcomes, and coordinate services to support their goals.

Key Responsibilities
  • Conduct assessments to identify the needs and goals of individuals with Intellectual Disabilities and/or Autism
  • Develop and implement Individual Support Plans that are person-centered and reflective of the individual's desired outcomes
  • Collaborate with service providers to locate and coordinate services that meet the individual's needs
  • Monitor the individual's health, safety, progress, and needs on an ongoing basis
Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree in a related field, such as Sociology, Social Welfare, or Psychology
  • Excellent time management and organizational skills
  • Strong communication and interpersonal skills
  • The ability to work independently and as part of a team
  • Computer proficiency and experience with electronic medical records
Work Environment

This full-time position offers a flexible 8-hour shift schedule, Monday through Friday. We also offer opportunities for hybrid work arrangements after successful completion of training.

Compensation and Benefits

We offer a competitive salary range of $45,000 - $60,000 per year, depending on experience, plus benefits including health insurance, retirement plan, and paid time off.
