JAG Physical Therapy is a comprehensive outpatient, orthopedic physical therapy company with 100 facilities throughout Pennsylvania, New Jersey, and New York.
We are seeking compassionate and motivated individuals to join our team as licensed physical therapists.
Job SummaryThe successful candidate will excel in patient assessment and developing customized treatment plans aimed at healing and functional independence.
This role requires strong clinical decision-making, discretion, and the ability to provide supportive patient care in a positive environment.
Therapists collaborate within an interdisciplinary team and ensure compliance with healthcare regulations and documentation standards.
About the JobAs a licensed physical therapist at JAG Physical Therapy, you will collect and record patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers.
This includes initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
You will also prepare for evaluations in accordance with department policies/procedures and standards of practice, selecting and administering tests and measures.
Synthesizing evaluation results, you will formulate diagnoses, goals, prognosis, and plan of care.
You will establish treatment plans based on evaluation results, goals, and best practices, collaborating with interdisciplinary teams, patients, and their support systems.
Additionally, you will assess and re-assess the need for modifications to the plan of care and goals.
